On your first visit we will ask you to bring completed forms with personal data and health background information. You can find these FORMS on this website, fill them out, and send them to us electronically, or print them out and bring them in for your first visit.
This information is designed to help us complete insurance forms or other correspondence that is required. Since we are concerned about your child’s total health, it also provides a baseline history to aid us in providing better dental and orthodontic care. Please keep us informed regularly of any changes in name, address, phone number, general health, and medications.
No one likes to be surprised. Anyone who is about to undergo dental treatment likes to have some idea of what the investment will be. We can prepare an estimate of your planned treatment based on the doctor’s evaluation of your wants and needs.
As a convenience for you, our office will submit fees for your treatment to your insurance carrier. We will be happy to complete the technical portion of filing claims and will accept payment on your behalf. We only ask that you carefully read your policy to be aware of any restrictions that apply to your benefits.
If you have any questions about your coverage, bring your policy or booklet and we will help figure out the details. We appreciate payment of your estimated portion at the time of service. By law, your insurance company is required to pay each claim within 30 days of receipt. We file all insurance claims electronically so your insurance provider will receive the claim the day of service.
You are responsible for any balance on your account after 30 days, whether insurance has paid or not. We will be glad to send you a refund once insurance has paid us. Please be aware that we file dental insurance as a courtesy to our patients. We do not have a contract with your insurance company — only you do. We are not responsible for how your insurance handles its claims or what benefits they pay on a claim.
Treatment estimates can help you decide which method of payment best fits your needs. Please feel free to discuss fees with us. We do request payment at the time of service for all visits. Payments can be made by cash, check, any major credit card, and through our secure website for your convenience.
Payment arrangements for extensive treatment as well as orthodontic treatment will be determined before the start of treatment. We have arrangements through CareCredit™ with competitive interest fees … often 0% for up to 24 months.
The time we have reserved in Dr. Anokhi and Dr. Derek’s schedule is just for you. Please let us know as far in advance as possible if you must change an appointment. If you cannot keep a scheduled appointment, 48 hours notice allows us to arrange for a patient with an emergency to be treated. (Someday it may be your child who needs that emergency appointment!)
When we are not in the office, our answering machine is always available for you to contact us about your appointments. We are always available for emergency calls after hours. We return all calls.
We have a number of ways to remind you of your upcoming dental appointments. These include e-mail reminders, text messages, and phone calls. Let us know which reminder works best in your busy lifestyle, and we will make the arrangements!
Through our preventive approach to dental care, the need for emergency treatment is greatly reduced. If an emergency does arise, please call, and we will arrange to see you as soon as you arrive at the office.
